Bovine colostrum has long been studied as a potential fortifier for human milk in premature and low-birth-weight infants. Rich in bioactive compounds like immunoglobulins and growth factors, it shows promise in supporting neonatal health. However, recent clinical trials have not delivered the expected improvements in growth or disease prevention. Researchers initially believed bovine colostrum could enhance gut barrier function and reduce inflammation in newborns. Its natural composition includes antimicrobial peptides and cytokines, which may benefit early development. Early postnatal administration was thought to increase receptiveness to these molecules.
Despite positive in vitro results, trials have not shown significant gains in growth velocity or reductions in necrotising enterocolitis (NEC) and sepsis rates. Concerns have also emerged about immune compatibility, as bovine immunological components may not fully match human neonatal needs. Allergenic reactions remain a potential risk.
Variability in sourcing and processing further complicates its use. Standardising manufacturing and quality control could improve consistency. Advanced techniques like proteomics and metabolomics are now being explored to better understand its molecular effects. Combining colostrum with prebiotics or probiotics might also enhance its impact on the neonatal gut. The full potential of bovine colostrum in neonatal care remains uncertain. Further research is needed to clarify its molecular interactions and optimise its use. Without standardised protocols, its clinical benefits may continue to fall short of expectations.
